Good Faith Estimate Notice

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your healthcare provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.

Cancellation Policy

If you are unable to keep your scheduled appointment, please cancel the appointment with a minimum of 48 hours in advance of your appointment time. If a minimum of 48 hours notice is not provided, you will be charged the full private pay session fee for the missed appointment. Insurance companies do not reimburse for missed appointments.

The only exception to the cancellation policy is in the event of a serious illness, contagious illness and/or exposure to COVID, or an emergency. Examples of emergencies include car accidents, deaths in the family, or extreme illness. Work issues do not constitute emergencies. In the event of adverse weather conditions, we can conduct counseling sessions virtually through a HIPAA compliant platform.

Wait Time

Due to the length of time provided for each session, it is important that you are on time for your appointment. If you are more than 10 minutes late to your appointment, we will have no choice but to reschedule your appointment and you will be responsible for the missed appointment fee as per the cancellation policy.
